Utilidad de la hemoperfusión con Polimixina B en la enfermedad pulmonar intersticial rápidamente progresiva asociada a anti-MDA5

Resumen

La dermatomiositis asociada a anticuerpos anti-MDA5 engloba diferentes fenotipos clínicos como son la vasculopatía cutánea, la afectación reumatológica y la enfermedad intersticial pulmonar rápidamente progresiva, la cual implica peor pronóstico. Esta afectación puede suponer un reto en la práctica clínica por la complejidad para llegar al diagnóstico y la refractariedad en los tratamientos.

Se recomienda iniciar una terapia combinada basada en glucocorticoides a altas dosis e inmunosupresores clásicos. En casos refractarios se han descrito otros tratamientos como la plasmaféresis, las inmunoglobulinas IV, tofacitinib o la hemoperfusión con Polimixina B.
