Síndrome TINU. Presentación abigarrada que dificulta el diagnóstico

El síndrome TINU es muy poco frecuente. Se trata de una nefritis intersticial aguda a la que sigue, meses después, una uveítis anterior aguda uni o bilateral. Está precedido por un síndrome pseudogripal. Su causa es desconocida, se especula con una enfermedad inmunomediada idiopática, precipitada por fármacos o infecciones. Está infradiagnosticada, ya que el tratamiento de la nefritis enmascara la uveítis. Una mujer de 55 años, con obesidad y diabética, consumidora de AINE, acudió a Urgencias por hipoglucemia y fracaso renal agudo. Al mes fue diagnosticada de uveítis derecha y al mes siguiente presentó uveítis izquierda. La presencia concomitante de fracaso renal y uveítis anterior bilateral llevó al diagnóstico de síndrome TINU. Con un tratamiento esteroideo a dosis altas desapareció la sintomatología ocular y la función renal se recuperó
a los 9 meses.
