Lymphoproliferative syndrome with positive Epstein-Barr virus and autoimmunity

We present a 75-year-old male with constitutional syndrome and lymphadenopathy. Studies showed organomegaly, bicytopenia, and positive lupus serology, creating a diagnostic dilemma between lymphoma and autoimmune disease. The condition was complicated by secondary hemophagocytic syndrome. The lymph node biopsy
was diagnostic for an EBV-positive B-cell lymphoma. This case underscores the importance of correct clinical and laboratory correlation to avoid diagnostic errors in complex conditions, where positive autoimmunity can be a confounding factor.
