Felty syndrome without arthritis

Among the peripheral causes of pancytopenia, hypersplenism is the most significant. We present the case of an 82-year-old woman admitted for orbital cellulitis in the context of pancytopenia for several months. A central cause was ruled out during the workup, and positive anti-cyclic citrullinated peptide antibodies as well as splenomegaly were identified. During hospitalization, the patient developed a symmetrical polyarthritis affecting the metacarpophalangeal joints, leading to a diagnosis of rheumatoid arthritis (RA) with Felty’s syndrome (FS). It is extremely rare for FS to occur prior to the diagnosis of RA. The response to methotrexate was favorable.
