Pulmonary nocardiosis and influenza A in an immunocompetent patient: a true diagnostic challenge

Abstract

Pulmonary nocardiosis is a rare opportunistic bacterial infection in immunocompetent patients that presents with nonspecific respiratory symptoms and radiological findings. Infection with the influenza A virus can promote infection by nocardia. The diagnosis requires high clinical suspicion and microbiological confirmation since the first-line treatment adjusted according to susceptibility is trimethoprim-sulfamethoxazole for 6–12 months to prevent relapse. We present the case of an 82-year-old woman with no history of pulmonary disease or immunodeficiency who arrived to the emergency department with dyspnea and dry cough as a manifestation of pulmonary infection by Nocardia cyriacigeorgica associated with influenza A infection.
