Acute myopericarditis as the initial manifestation of an adult-onset Still’s disease flare

Adult-onset Still’s disease (AOSD) is a rare systemic autoinflammatory disorder. We present the case of a 30-year old man who initially presented with acute myopericarditis with severe biventricular dysfunction, accompanied by a generalized elevation of proinflammatory markers, extreme hyperferritinemia, and an atypical skin rash. After infectious and autoimmune etiologies were ruled out, he was diagnosed with an AOSD flare and treatment with corticosteroids and immunomodulatory therapy was started, with prompt clinical improvement. This case is notable for its initial presentation, given the rarity of myocardial involvement as the first manifestation of the disease, and highlights the importance of correctly interpreting inflammatory markers within the patient’s clinical course and overall context.
