Lemierre syndrome: a diagnostic and therapeutic challenge in the intensive care unit

Abstract

Lemierre's syndrome is a rare complication of oropharyngeal infections, characterized by jugular thrombophlebitis and septic emboli. We present the case of a 21-year-old female, a vape user, admitted for respiratory sepsis with multiple pulmonary embolisms and internal jugular thrombosis. Despite presenting with negative cultures due to prior antibiotic therapy and initial severe thrombocytopenia, she progressed favorably thanks to ventilatory support, broad-spectrum antibiotics, and anticoagulation. In conclusion, we wish to highlight that a compatible clinical-radiological presentation confirms the diagnosis despite sterile cultures. Furthermore, vaping emerges as a potential predisposing factor by altering the oropharyngeal barrier.
