Comprehensive management and VExUS-guided decongestion in the frail elderly patient with polytrauma

A 93-year-old woman was admitted with a supracondylar femur fracture, presenting with severe anemia and mild
renal impairment on admission, requiring blood transfusion, intravenous fluid therapy, and surgical treatment. In the postoperative period, she developed acute respiratory failure and worsening renal function with oliguria. Hemodynamic assessment by the Internal Medicine team using the VExUS protocol revealed signs of systemic venous congestion consistent with congestive nephropathy secondary to fluid overload. Discontinuation of intravenous fluids and initiation of diuretic therapy led to recovery of renal function. The comprehensive approach provided by Internal Medicine also facilitated the management of other associated medical complications.
