A 89 year-old woman hospitalized for left hip fracture. During the postoperative period there was deterioration of the general state with temporo-spatial disorientation, agitation, arterial hypotension, tachycardia and tachypnea, with active bleeding in the surgical wound area, with impaired renal function, hydroelectrolytic alterations, anemization and arrhythmia development, and secondary heart failure. After electrolyte replacement and red blood cell transfusion, the patient presented improvement with cessation of delirium symptoms. The identification of the precipitating causes of delirium, as well as the early action on the risk factors of delirium, are essential to achieve therapeutic success.
