Prolactinoma in a young patient with constitutional symptoms

Abstract

Pituitary prolactinomas are the most common macroadenomas. They can cause symptoms due to the mass effect of the tumour or due to the activity of the secreted hormone itself. The main symptom presented by the patient described below was only asthenia. After requesting a laboratory test with a basic hormonal profile (TSH, FT4, and basal cortisol), we detected central hypothyroidism and adrenal insufficiency. Based on these data, we requested a pituitary MRI with findings of a pituitary macroadenoma. Further laboratory tests by the endocrinologist revealed very elevated prolactin levels. The macroadenoma was finally removed by transsphenoidal surgery.
