Abstract

We present the case of a young man with persistent back pain and constitutional symptoms. Imaging studies
revealed multiple lytic bone lesions and a paravertebral abscess. Initial suspicion was metastatic disease. Histopathology
showed granulomatous inflammation and cultures confirmed Mycobacterium tuberculosis. Antitubercular
treatment was initiated with good clinical evolution. This case highlights the importance of considering
osteoarticular tuberculosis in the differential diagnosis of lytic lesions, even in young, immunocompetent patients
without pulmonary involvement.
